Chances are, you don't have the actual Fertility HUD. All PsiCorp v5.0 bits come with basic fertility, this is to allow (broadly speaking) males to impregnate (broadly speaking) females.
If you wish to become Fertile, you'll need to purchase the actual Fertility HUD which will give you the full options and settings to better enhance your roleplay.
